When you should receive a standard choice form

If you are eligible for Super Choice, your employer is obliged to provide you with a standard choice form:

  • within 28 days of commencing employment, including new NAPSA employees employed from 1 July 2006
  • within 28 days of being requested by you, provided you haven't made such a request in the previous 12 months
  • when your employer becomes aware that they are unable to contribute to your chosen fund
  • when you have chosen a fund that's no longer an eligible choice fund (ie. the fund has ceased to be a complying fund), or
  • when your employer changes their employer fund.
