By ticking this box you consent to Host-Plus Pty Ltd ABN 79 008 634 704, AFSL No. 244392, RSEL No. L0000093, as the trustee of Hostplus Superannuation Fund ABN 68 657 495 890, RSE No. R1000054 (Hostplus) using your tax file number (TFN) to search Australian Taxation Office (ATO) records to locate superannuation amounts held on your behalf by other superannuation funds or by the ATO now and in the future.

If search results include superannuation amounts held on your behalf by the ATO, you also consent to Hostplus transferring these amounts into your Hostplus account and using and disclosing your TFN for that purpose.

Tax file number (TFN)

We are authorised to collect your TFN under the Superannuation Industry (Supervision) Act 1993. You are not obliged to disclose your TFN, however there may be consequences in not providing it. These include being subject to additional tax, as well as Hostplus being unable to accept some contribution types and to consolidate multiple super accounts you may have.

By providing your TFN you give your consent for the Trustee of Hostplus to use it to search for multiple superannuation accounts with the ATO or other superannuation funds or Retirement Savings accounts. If you consent below, we can also use your TFN to help you find super you have lost.

Privacy

We appoint third parties to help us administer your account and authorise these parties to collect and use your information on our behalf. Third parties are appointed only when they can satisfy certain privacy and confidentiality requirements. We have appointed Link Group (ABN 81 615 082 534) as our service provider to deliver SuperMatch2 services.
